Simple Project - An application that collects a row of information (think of a row in an excel spreadsheet) and has one or two reports based on that information. The reports can be exported to an excel spreadsheet (allowing for further analysis outside the application).
The user volume or transactional volume has no bearing on the Tier. If you have a simple interface (as described above), but have thousands of users we still consider that a Tier 1 project.

$100/month + a per transaction or per user fee.
For example it could be $100/month + $10/user/month.
Whether it is user or transactional will depend on your application requirements and will be determined in advance of development.
There will be no obligation to pay the monthly fee unless you actually use the application during the month (thus risk free).

An application that allows sales reps to enter sale information daily. The reps would log into the web site with their user name and password (this could be a computer based web site or a wireless phone based web site or both) and enter in sales numbers of different product types. One or two summary reports could be viewed by Managers of those sales reps and the data could be exported to excel.

An application that dispatches jobs to technicians via text messages to a cell phone. The technicians can then accept the job, letting the home office know they plan to perform the work. The job will then show up in the technicians list of open jobs and they can indicate when the job has been started and when the job has been completed. The home office can see all activity in real time as well as view a number of historical reports based on when jobs were received, started, and completed.
